Our Collaboration

FAHWA Charity have outreach programs regularly to the consumers of St Barts. These programs are mobile assessment screening for diseases and symptoms. We provide health promotion and education on Medication, Diabetes education and checks, Skin checks, Continence, Hygiene education and Men’s and Women’s health.

St Bart’s has stood firmly by the side of vulnerable West Australians experiencing, or at risk of homelessness, offering mental health support, housing and aged care services to those who need them most. They offer secure accommodation, support services, as well as intensive support for people experiencing, or at risk of homelessness.

St Bart’s Mental Health Support Services coordinate accommodation for people living with a mental illness who need 2-4 hours support per day in order to live independently.

 

FAHWA Inc  trained  nurses volunteer  to accompany  these residents with tracheostomy out on a bus outings  around the city regularly.  Other non-clinical members volunteers one hour a day to act as companion to chat, play games such as scrabble and walk around  the gardens.

FAHWA’s aim is to help SwanCare residents quality of life for those who have limited family, cultural and social contact and maybe at risk of social isolation.  Also to support SwanCare staff to provide a level of empathy and professionalism that ensures their residents’s needs, comfort and dignity are preserved in every interaction.

SwanCare is a non- profit WA residential aged care for seniors who can no longer live independently at home and for residents who require a higher level of care.  This because of physical or mental limitations, personal safety concerns, or increasing health and daily living needs. Services includes clinical care services for complex care needs, such as tracheostomy care.
